Pennsylvania Advance Directive Form

Below is the information you'll need in order to obtain an Advance Directive in Pennsylvania.

Overview: The Pennsylvania Living Will is called "Healthcare Treatment Instructions" and is included in an Advance Directive packet that also contains a form for naming a "Durable Power of Attorney for Healthcare" (a.k.a. Health Care Proxy) and specifying Organ Donation.

Required Information: You and two (2) witnesses must sign your PA Advance Directive for it to be considered valid.
