North Dakota Death Certificate

Below is the information you'll need in order to obtain a death certificate in North Dakota, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

The following people may request death certificates from the state of North Dakota:

  • Spouse
  • Parent or legal guardian
  • Child
  • Grandparent
  • Grandchild
  • Authorized representative
  • The child fatality review board
  • Funeral director reporting the facts of death
  • The order of a court of competent jurisdiction

Cost

The fee to search for a death certificate is $5.00, which includes one certified copy of the death certificate. For each additional copy of the certificate ordered at the same time, the fee is $2.00. Shipping via FedEx or UPS is an additional $16.00 (plus $6.00 for delivery to Alaska or Hawaii). Checks or money orders should be made payable to the "ND Department of Health." Credit card payment is accepted (Visa, MasterCard, Discover). Cash is not accepted. Fees are not refundable.

You may request a certified death certificate in person by going to the Division of Vital Records, located at:

600 E. Boulevard Avenue, Room 118

Bismarck, ND 58505-0200

Room 118 is located in the Judicial Wing of the State Capitol Building. The office is open from 7:30 am until 5:00, Monday through Friday, except for holidays. Walk-in customers can usually get their certified copy with in a few minutes after providing photo ID and filling out the request form.
