Kentucky Death Certificate

Below is the information you'll need in order to obtain a death certificate in Kentucky, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

There are no restrictions on who may request certified death certificates in Kentucky.

Cost

The fee to search for a death certificate is $6.00, which includes one certified copy of the death certificate. Each additional copy of the record is also $6.00. Checks or money orders should be made payable to the "Kentucky State Treasurer." Cash is not accepted. Fees are not refundable.

  • Funeral service provider
  • Your relationship to the deceased
  • Your name
  • Your address
  • Your phone number
  • The number of copies being ordered

Submitting Your Request

By Mail:
Send the completed application and the appropriate fee to:

In Person:
You may order a certified copy of the death record by going in person to the Office of Vital Statistics, located at:

Orders are accepted for same-day issuance between 8:00 am and 3:30 pm, Monday through Friday.
