Delaware Death Certificate

Below is the information you'll need in order to obtain a death certificate in Delaware, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

The following people may request death certificates from the state of Delaware:

  • Husband, wife, or civil union spouse
  • Child
  • Parent
  • Legal guardian
  • Authorized agent, attorney, or legal representative of the deceased

All applicants must provide a copy of valid photo identification (driver's license, state ID, work ID). Parents requesting death certificates must provide identification proving that they are the parents.

Cost

The fee to search for a death certificate is $25.00, which includes one certified copy of the death certificate or a "Certificate of Failure to Find." Checks or money orders should be made payable to the "Office of Vital Statistics." Cash is not accepted. Fees are not refundable.
