Oklahoma Death Certificate

Below is the information you'll need in order to obtain a death certificate in Oklahoma, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

There are no restrictions on who may request certified death certificates in Oklahoma.

Cost

The fee to search for a death certificate is $15.00, which includes one certified copy of the death certificate. Each additional copy of the record ordered at the same time is also $15.00. Checks or money orders should be made payable to "OSDH." Cash is not accepted. Fees are not refundable.

  • Your name
  • Your phone number
  • Your address
  • Reason for requesting the certificate
  • Photocopy of current, government-issued photo ID

The following information is optional, but may help in locating the record:

  • Deceased's social security number
  • Gender
  • Spouse's name
  • Date of birth
  • Place of birth
  • Father's name and birthplace
  • Mother's name and birthplace
