Qualifications

The following people may request death certificates from the state of West Virginia:

  • Parents
  • Grandparents
  • Spouse
  • Children
  • Grandchildren
  • Siblings
  • Funeral director
  • One with legal interest in the certificate

Cost

The fee to search for a death certificate is $12.00, which includes one certified copy of the death certificate. Each additional copy of the record ordered at the same time is also $12.00. Checks or money orders should be made payable to "Vital Registration." Cash is not accepted, except if the request is made in person. Fees are not refundable.

In Person:
You may request a certified death certificate in person for the office of Vital Registration, located at:

350 Capitol Street
Charleston, WV 25301-3701

The public entrance to the building is on Capitol Street, and the office of Vital Registration is located on the right side of the lobby. The office is open from 8:30 am until 5:00 pm, Monday through Friday, excluding state holidays. Same-day service is available.
