Utah Death Certificate

Below is the information you'll need in order to obtain a death certificate in Utah, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

The following people may request death certificates from the state of Utah:

  • Parent
  • Sibling
  • Spouse
  • Child
  • Grandparent
  • Grandchild
  • Persons with written permission from the immediate family

Cost

The fee to search for a death certificate is $16.00, which includes one certified copy of the death certificate. For each additional copy of the certificate ordered at the same time, the fee is $8.00. Checks or money orders should be made payable to "UCHD." Cash is not accepted. Fees are not refundable.

The Salt Lake City Vital Records office has records for death in Salt Lake County beginning from September 1969. Records also include computer death certificates for all Utah deaths from August 2006 to the present.

Ogden County:

The Ogden County Vital Statistics Office has records for death in Ogden and Weber County, from 1958 to present; Morgan County, from 1993 to present; and State of Utah copies, from 2006 to the present.

The office is open from 8:00 am until 5:00 pm, Monday through Friday. Requests taken after 4:30 pm will be processed the next business day.
