Florida Advance Directive Form

Overview: The Florida Living Will is included in an Advance Directive packet that also contains forms for naming a Health Care Surrogate (aka Health Care Proxy) and specifying Anatomical Donation.

Required Information: You and two (2) witnesses must sign your FL Advance Directive for it to be considered valid. One (1) of the witnesses CAN NOT be your husband, wife or blood relative
