Connecticut Death Certificate

Below is the information you'll need in order to obtain a death certificate in Connecticut, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

Connecticut death certificates are available to the general public.  Anyone over 18 years of age may request a copy of any Connecticut death certificate.

If you are requesting a death certificate for a death that occurred after July 1, 1997, the social security number of the decedent will not be included on the certificate that you receive unless you are:

  • Immediate family member of the decedent
  • Executor of the decedent’s estate
  • A party specified on the death certificate, such as the informant, funeral director/ embalmer, or certifying physician
  • Other persons as authorized by Department of Public Health

For any death occurring after December 31, 2001, the “Administrative Purposes” section of the death certificate, which includes the decedent’s social security number, occupation, business/industry, race, Hispanic origin, and educational level, will not be included on the certificate you receive, except to the above mentioned parties.

Cost

The fee to search for a death certificate is $20.00 and includes one certified copy of the death certificate or a "Certificate of Failure to Find." If you are ordering a death certificate from the town or city where the death occurred, send a money order made payable to that town or city. If you are ordering a death certificate from the State Vital Records Office, send a money order or bank check made payable to “Treasurer, State of Connecticut.” Cash and personal checks are not accepted. Fees are not refundable.
