Tennessee Death Certificate

Below is the information you'll need in order to obtain a death certificate in Tennessee, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.


The following people may request death certificates from the state of Tennessee:

  • Parent
  • Legal guardian
  • Child
  • Spouse
  • Legal representative of the deceased


The fee to search for a death certificate is $7.00, which includes one certified copy of the death certificate. Each additional copy of the certificate ordered at the same time is also $7.00. For expedited requests there is an additional fee of $5.00. Checks or money orders should be made payable to "Tennessee Vital Records." Cash is not accepted. Fees are not refundable.

  • Name of funeral home
  • Location of funeral home
  • Your relationship to the deceased
  • The purpose of the certificate
  • Whether you want a certificate with cause of death information or not
  • Your phone number
  • Photocopy of current, government-issued photo ID

Submitting Your Request

By Mail:
Send the completed application and the appropriate fee to:

If you are requesting expedited service, include a note requesting expedited service along with the additional $5.00 fee and a pre-paid self-addressed shipping envelope.

In Person:
You may request a certified death certificate in person from the Vital Records office, located at:

Central Services Building, 1st Floor
421 Fifth Avenue North
Nashville, TN 37243

The office is open from 8:00 am until 4:00 pm on Mondays, Wednesdays, and Fridays. The office is open from 8:00 am until 6:00 pm on Tuesdays and Thursdays.
