Below is the information you'll need in order to obtain an Advance Directive in Tennessee, which is comprised of two separate documents.

Overview: In Tennessee you must fill out two separate forms to have a complete Advance Directive: A Living Will called "Advance Care Plan" and a "Healthcare Agent" (a.k.a. Health Care Proxy).

Required Information for the Living Will: Your signature must be Notarized OR two (2) witnesses must sign your TN Living Will for it to be considered valid. Witnesses CAN NOT be the person appointed as your agent or alternate, AND at least one of the witnesses must be someone who is NOT related to you or entitled to any part of your estate.

Required Information for Naming a Health Care Proxy: Your signature must be Notarized OR two (2) witnesses must sign your TN Health Care Proxy form for it to be considered valid.
