New York City Death Certificate

Below is the information you'll need in order to obtain a death certificate in New York City (The Bronx, Brooklyn, Manhattan, Queens, Staten Island), including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Note: This information only applies to the five boroughs of New York City (The Bronx, Brooklyn, Manhattan, Queens, Staten Island). For a death that occurred elsewhere in New York state, click here.


The following people may request death certificates from the five boroughs of New York City:

  • Spouse
  • Blood relative of the deceased
  • One applying on behalf of an authorized requestor with a notarized statement
  • One who can establish a right to obtain this document

Applicants not related to the deceased will need to establish a right to obtain a death certificate by providing any of the following original or certified documents, which include both your name and the name of the deceased:

  • Insurance policy
  • Will
  • Bank book or statement
  • Property deed
  • Other document showing entitlement


The fee to search for a death certificate is $15.00, which includes one certified copy of the death certificate or a Not Found Statement. For each additional copy of the certificate ordered at the same time, the fee is $3.00. Checks or money orders should be made payable to the "NYC Department of Health and Mental Hygiene." Cash is not accepted. Fees are not refundable.

  • Photocopy of current, government-issued photo ID
  • Notarized signature (if application submitted by mail)

The office is open from 9:00 am until 3:30 pm, Monday through Friday. Use the Lafayette Street (handicapped accessible) or Centre Street entrances. Lines are shortest in the morning.
