New Mexico Death Certificate

Below is the information you'll need in order to obtain a death certificate in New Mexico, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.


The following people may request death certificates from the state of New Mexico:

  • Parent
  • Sibling
  • Child
  • Current spouse
  • Maternal grandparent
  • Paternal grandparent is eligible if father is listed on the vital record
  • Those who represent tangible proof of legal interest in the requested record


The fee to search for a death certificate is $5.00, which includes one certified copy of the death certificate. Each additional copy of the record ordered at the same time is also $5.00. Certified checks or money orders should be made payable to "New Mexico Vital Records." Cash is not accepted. Fees are not refundable.

You may request a certified death certificate in person by going to the Santa Fe State Office, located at:

1105 South St. Francis Drive
Santa Fe, NM 87505

The office is open from 8:30 am until 4:00 pm, Monday through Friday.
