New Hampshire Advance Directive Form

Below is the information you'll need in order to obtain an Advance Directive in New Hampshire.

Overview: The New Hampshire Living Will is included in an Advance Directive packet that also contains a form for naming a Health Care Proxy.

Required Information: You and two (2) Witnesses, a Notary Public or Justice of the Peace must sign your NH Advance Directive for it to be considered valid.
