Indiana Death Certificate

Below is the information you'll need in order to obtain a death certificate in Indiana, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

Indiana law specifically requires that the person who purchases a death certificate has a direct interest, which is defined as a documented personal financial or legal interest in the record. This also includes immediate family members, specifically:

  • Parents
  • Grandparents
  • Adult siblings

Cost

The fee to search for a death certificate is $8.00, which includes one certified copy of the death certificate. For each additional copy of the certificate ordered at the same time, the fee is $4.00. Checks or money orders should be made payable to the "Indiana State Department of Health." Cash is not accepted. Fees are not refundable.

  • Date of birth of deceased
  • Name of father
  • Maiden name of mother
  • Your relationship to the deceased
  • Your name
  • Your mailing address
  • Your phone number
  • A photocopy of a current, government-, state-, or military-issued identification

The address provided on the application must match the address on the identification or the request will not be processed.
