Arizona Advance Directive Form
Below is the information you'll need in order to obtain an Advance Directive in Arizona.

Overview: The Arizona Living Will is included in an Advance Directive packet that also contains a form for naming a Health Care Proxy.
Required Information: Your signature must be Notarized OR one (1) witness must sign your AZ Advance Directive for it to be considered valid. The witness must be at least 18 years of age and CAN NOT be family (related by blood, adoption or marriage), in your Will to receive part of your estate, appointed as your representative or a health care giver.
