Overview: The West Virginia Living Will is called "Health Care Directive" and is included in an Advance Directive packet that also contains a form for naming "Medical Power of Attorney" (a.k.a. Health Care Proxy).
Required Information: Your and two (2) witnesses' signatures must be Notarized for your WV Advance Directive to be considered valid. Witnesses MUST BE at least 18 years of age and NOT related to you by blood or marriage.
